TMD Splints

TMD splints — also known as TMJ splints, occlusal splints or bite splints — are dental appliances used to help manage temporomandibular disorders (TMD).

⭐ What are TMD splints:

A TMD splint is a custom-made acrylic appliance placed over the upper or lower teeth. It is designed to:

  • Reduce the load on the jaw joint (TMJ)
  • Reduce muscle tension
  • Prevent teeth grinding or clenching
  • Improve jaw alignment during function

🦷 Types of TMD splints

1. Stabilization splint (flat night splints)

  • Most commonly worn at night
  • Covers all upper or all lower teeth

2. Anterior bite splint / anterior “deprogrammer”

  • Makes contact only with the front teeth
  • Reduces the intensity of clenching
  • Used short-term (long-term wear is not recommended due to possible changes in tooth position)

3. Repositioning splint (splint for mandibular repositioning)

  • Guides the jaw into a slightly different position
  • Used when there is disc displacement or impaired joint function
  • Usually worn for a limited period to avoid changes in the bite

4. Soft splint

  • Made of flexible material
  • More comfortable, but in some individuals it may increase clenching

1. Orthodontic retainers

These are most commonly used after wearing braces to prevent tooth movement. They are not specifically intended for TMD treatment, but some types — especially clear, removable ones — may feel like a splint if they change the way the teeth contact during biting.

In some individuals, a feeling of pressure, clicking, or tension in the jaw may occur when wearing regular retainers, especially if they have a habit of clenching or grinding their teeth.

Key:

  • Traditional retainers maintain tooth position; they do not treat TMD
  • Sometimes they may make joint symptoms more noticeable if they change the bite contact

Self-ligating braces

⭐ What are self-ligating braces?

Self-ligating braces are a type of orthodontic brackets that do not use elastic ligatures (rubber bands) to hold the wire in place. Instead, each bracket has a built-in clip or sliding mechanism that secures the wire.

✔ Advantages:

  • Less friction between the wire and the brackets → potentially smoother tooth movement
  • Shorter adjustment appointments since there are no elastic ligatures that need to be replaced
  • Easier maintenance and cleaning, since there are no elastic ligatures that trap plaque and debris
  • Some patients experience slightly less discomfort during tightening / adjustment visits

✔ Types:

There are two main mechanisms:

  • Passive self-ligating — the clip does not press tightly on the wire (often used in the initial alignment phase)
  • Active self-ligating — the clip applies gentle pressure on the wire (useful for greater control)

Self-ligating braces

What are metal braces?

Metal braces consist of:

  • Stainless steel brackets that are bonded to the teeth
  • An orthodontic wire (archwire) that passes through the brackets
  • Elastic ligatures or self-ligating clips that hold the wire in place

Through controlled pressure, they gradually move the teeth into the correct position.

Advantages:

  • ✔ Usually more affordable than ceramic or “clear” options
  • ✔ Modern versions are smaller and more comfortable than before

Disadvantages:

  • ✖ Require careful oral hygiene to prevent plaque buildup
